Assessments and questionnaires are different things

A questionnaire is the set of questions. An assessment is one instance of asking a client to complete a particular published version of it, with its own name, recipients, and dates. One questionnaire can be behind many assessments; each stays pinned to the version it was created with, so publishing a newer version never changes what an open assessment is asking.
